Picked up a copy of this 1980 FASA product. Some questions for the experts:

That looks like an illustration by William H. Keith, Jr. on the cover but there's no signature or credit. Is that his?

The drawings all say "Revised DCW-JKW". I assume JKW is Jordan Weisman. Who is DCW?

Unfortunately, it looks like I got a copy of the insert text. Can anyone confirm that this should be a single sheet with Situation One on the front and Situation Two on the back? Were there any other sheets I'm missing (besides the covers of course)?

The cover was a single sheet folded. Stats and art on the front, Situation three on the back. Sit one and two are on the inside. There was also a single sheet of the same tan cardstock with a lineart side view and a repeat of the stats on one side and actual rear cover copy on the other.

The five double-size sheets of plans complete the set.
