Exactly what I was looking for.

I have requested to use this BEAUTIFUL form in my campaign and modify
it to my tastes.

With permission I will show it with the new the modifications I created.

Its original is by "ravells" here on Citizens of the Imperium.

Found the maker of this form on another site... he still has the same name "ravells" and he is good with the re-posting / re-making of his original form. So.... here it is.

Any chance the B&W version could be turned into a PDF? (Preferably fillable?)

If ravells gives permission, yes, a fillable PDF isn't out of the question.

Since he gave his permission I started creating it using Word. Here it is after just getting a feel for the layout. I am making it to fit on 11x17 cause I just feel that is a good size for it, but if you want something different, let me know.

And I am not all that great with photoshop so if anyone wants to make fancy graphic parts to put on here, feel free and I will include them.
