Do armor 'items' stack with armor systems? Can I wear a quilt 'item' underneath battle dress? Can I wear a heavy coat 'item' over assault armor? Do the armor values stack? Does a shield stack with battle dress, or only with other 'items'? What exactly is an advanced shield and why does it protect against radiation?

Most of those questions are not T5 questions, but rather, simply Traveller questions. Players have asked that question of Traveller referees, probably since 1977.
...To which the answer is "ask your referee", because he has final say. And if you are the referee, then it's up to you.
As for the final question, I find it is more productive to ask you what you think an advanced shield is, such that it protects against radiation?

Thanks for the reply. However, these are only T5 questions. I bought the books and I'm refereeing a campaign and have these questions about T5 specifically.
The armor system is different from other versions of Traveller I've played, such as the original back in the 80s and the Mongoose version, so they're no help here. I get why you state people have asked questions like this for decades, but T5 is quite a bit different in how its systems work. For example, high tech cloth states it can be worn under armor in Mongoose, but this 'armor stacking' question isn't addressed at all in T5. I can't help but notice rule #2 of your Loren Wiseman quote "Be careful not to destroy the balance of play." Indeed, and why these questions are important, especially because of quote #1!
Anyway, I see you're a Super Moderator, so that probably means you know T5 and other versions quite well. Your answer that everything is rule 0 makes me sad, but at least I can assume that there isn't any guidance provided in the books.

Ah, now I know why you were asking. It's because you saw this written in Mongoose, but it's not written in T5.
It's safe to say that if the rule says you can stack it, then you can stack it, otherwise you can't.
For example, reflec, p121 in Book 3 for example:
So "stacking" in a way is allowed for reflec. Sounds like you could even have multiple layers of it -- one over the armor, one over the clothes, and one under clothing.
But no such allowance is given for other armor types.
You're quite correct to consider Rule 2. To enable stacking would have to be a referee fiat, and unless managed by the referee could indeed unbalance the game. So yes, Rule 2 always applies when the referee adds to or amends the rules.
